SurveyAppMaker (SAM) offers a drag and drop interface through which to easily and quickly create and deploy digital survey and questionnaire forms. Once the survey is created, it appears automatically and ready for deployment on mobile app MobileSurveyForm.

Ideal for tailoring versions of the same survey form for use in different locations and circumstances, or by different populations. Indispensable for emergency response interventions and quick deployment anywhere in the world.

Data collected through MobileSurveyForm are automatically integrated into the client’s customized web engine platform described above and are immediately available for analysis and visualization

Android and iOS mobile app to collect official data using survey forms created on Web Engine. MQF was developed with an eye towards data pertaining to entomology, clinical epidemiology, social determinants, and other public health conditions, but it is a universal app to collect any type of data.
When internet connection is absent, data collected through MyQueryForm will be stored on the device. Once connectivity is restored, data records are sent to the server using secure encryption algorithms.
